Requirements

Age - In order to deploy with the Dayton DMAT, you must be 21 years of age. 

Physical/Mental - In order to deploy, a member should be physically and mentally up to the challenge.   Deployments can inflict a great deal of physical and mental stress on a member.  A fit body and a level head go a long way in helping a member support his team while deployed.  A history that includes the ability to work well with others is required. 

Federal Employee - All applicants must be accepted by the National Disaster Medical System for intermittent employee status. This is not a trivial chore!  Applicants must pass a background security check, and file many forms. FEMA courses  IS  100, 200, 700, and 800B must be also completed before an application is sent on to the NDMS for processing.  Each member will receive proper identification from the NDMS before they may deploy. 

Team Process - An Applicant must be qualified for the position requested and meet the above qualifications. Those wishing to join DMAT must be approved by DMAT leadership and NDMS.    Applicants will be denied membership on the team due to a failure to pass a back-ground check.  Each member is also required to maintain required vaccinations.

Field Exercise - All members who wish to deploy must first attend a week-end field training exercise that includes an over-night stay.

DMAT team members are expected to continue as active and supportive members by participation in team activities such as warehouse work, planning meetings and trainings.

Position Classification

When joining the Dayton DMAT, you will need to select a job classification that most closely describes your function on the team.  The team administrator can also assist you in identification of a proper classification.  The following is a partial list of job classifications available:

Health Tech EMT-I 640-06 EMT-I license and 1 year experience.
Health Tech EMT-P 640-07 EMT-P license and 2 years experience.
Health Technician Leader 640-09 3 years specialized experience or 1 year of experience with master's degree.
LPN / Vocational Nurse 620-05 Graduate of a school of nursing, current license, and 1.5 years experience.
Logistics Coordinator 303-07 2 years of general office work experience and 1 year of experience related to DMATs.
Medical Records Technician 675-05 1 year experience with occupation or related field.
Medical Supply Technician 622-05 2 years experience with occupation or related field.

645-05 One year experience with occupation.  Certification or current license.
Mental Health Specialist 101-11 PhD or 3 years of graduate education in social science field or masters and 1 year work.
Nursing Assistant 621-4 At least 3 months of experience in field.
Nursing Assistant Surgical Tech 621-3 At least 6 months of experience in field.
Nursing Assistant Stretcher Bearer 621-2 At least 3 months of experience in field.
Nurse Practitioner 610-11 Current License and 2 years experience and certification as nurse practitioner.
Pharmacist 660-11 4 year course of study and 1 year of internship and 1 year of professional experience.
Pharmacy Tech 661-05 1 year experience as pharmacy tech or assistant.
Physician Assistant 603-11 2 years experience at a PA.
Respiratory Therapist 651-07 1 year experience as a respiratory therapist and bachelor's degree.
Senior Medical Officer 602-14 MD or DO with current license.  3 years experience post graduate. Can include residency
Staff  Medical Officer 602-13 MD or DO with current license.  1 year experience post graduate. Can include residency
Staff  Nurse 610-05,06,07 Current license with 1 to 3 years experience.
Supervisory Clinical Nurse 610-11 Current license with 3 to 4 years experience.  Experience in supervising a nursing staff.
Telecommunications Specialist 0391-07 One year experience with Telecommunications, etc.
Telecommunications Specialist 0391-09 One year experience as 0391-07 Telecommunications Specialist
Telecommunications Specialist 0391-12 One year experience as 0391-09 and appointed Communications Officer

DMAT OH-5 Applicant Instructions

DHHS has announced the new application process for pending and new applicants.  It will be a 3 step process as follows:


FIRST STEP
:

1)      OF 612 Optional Application for Federal Employment complete or with resume.  If you submit just a resume, it must include all the information requested on the OF 612 form.  (Form 612 is only optional, if all of the requested information is in the resume.)  Include hours worked per week for each position, current and past.  Include detail of your work experience in your current job and past positions.  Section F must be completed.  DHHS will check to see is males are registered in the Selective Service.  If you answer YES to Section F4 you must include your final SF50 form.  BE SURE TO SIGN and DATE the form OF 612 and/or resume.

2)      NDMS Member Information form.

3)      Copies of education degrees and licenses for medical positions

4)      Direct Deposit Form – Attached voided check for financial institution info

5)      NEW REQUIREMENT:  Provide copies of FEMA Certificates of Completion for IS-100 IS-200, IS-700 and IS-800B NIMS courses. You can complete these courses online:  http://training.fema.gov

6)      MAIL ALL THE COMPLETED FORMS TO:

SECOND STEP:

1)      When your OF 612 application from step one is approved by DMAT OH5 and DHHS you will be required to be fingerprinted and to complete the Personal Identity Verification (PIV) Form.  DMAT OH-5 Management Team will send these completed forms to DHHS.

2)      You will receive an email from psc.hhs.gov to complete e-QIP – this is your background check information. 

YOU HAVE ONLY 5 DAYS TO RESPOND to this email! It is important that you meet that deadline, or if you cannot, to communicate that fact immediately to the point of contact who sent you the notification.

Follow all the suggestions in the instructions regarding your PC settings to complete this online requirement.    Based upon this process requirement, if you do not complete the e-QIP process within the allotted two attempts, you will be required to reapply to NDMS as a NEW Applicant.

 The three (3) e-QIP Signatory Forms must be completed, (e-qip Document type CER, REL, and MEL) as titled on lower right corner of the forms.

THIRD STEP:  After your background check is approved

1)      E Induction:  You will be notified by via email to complete the On-Line Induction process.  After completing the forms listed below, you will meet with team leadership who will review and approve.  Contact Patricia K. c)      Appointment Affidavit (do not sign) – Must be witnessed and be validated by team leadership

f)       Education Data Update Form

g)      Race & National Origin

h)      Record of Home Address

i)        Self identification of Handicap

 
All forms can be faxed or scanned and emailed, EXCEPT the W-4 and I-9 -- these two forms must be mailed by team management.

FINAL STEP:  HHS Credit Card Application

We realize this process can be a bit overwhelming, but stick with it, and do a bit at a time. You may also wish to use team personnel to help mentor you through the process. Please also remember that this process is not a speedy one and that it may take many months for all the peices to come together and to be processed through to completion.
